Clubhouse Rules

A completed family waiver must be submitted prior to participation in any clubhouse activity.

1. Select the button below which takes you to the Waiver;
2. Enter child/ren’s information on page two;
3. Select “New minor athlete” button on the page;
4. Enter your (parent) information on page one;
5. Read waiver carefully and check each box;
6. Enter Emergency Contact Information;
7. Sign in the box provided;
8. and click “Accept”
